nom patronymique le cas èchèant

Trying to interpret a declaration document sent to my wife and, and I'm not quite sure on the following (nom patronymique le cas èchèant). I am assuming that it means family name, but in the case of my wife would that mean "maiden name" ?

Le nom patronymique est celui qui vous été donné à votre naissance, so your wife's maiden name, le cas échéant.(where appropriate, if appropriate, if need be)
